How to structure it

  • Find out the patient’s prior understanding and specific concerns.
  • Explain AF and the increased stroke risk in plain terms.
  • Explain how anticoagulation reduces that risk, balancing stroke against bleeding risk.
  • Address their fears directly and discuss monitoring and follow-up.
  • Support a genuinely shared, informed decision.

Common pitfalls

  • Using jargon such as CHA2DS2-VASc without explaining it.
  • Delivering a monologue rather than a dialogue.
  • Pressuring the patient instead of sharing the decision.
