Your first NHS job after PLAB: what to expect

8 June 2026 2 min readLast reviewed 8 June 2026

With GMC registration in hand, the final step of the PLAB route is finding and starting your first NHS job. The clinical knowledge is the easy part; settling into UK systems, teams and ways of working is what takes adjustment. Knowing what to expect smooths the transition.

Types of first post

Many IMGs start in a trust-grade role (often badged as an FY2-equivalent or junior clinical fellow) rather than entering formal training immediately. These posts build UK experience and references that open the door to training pathways later.

Finding a post

  • NHS Jobs and trust websites advertise non-training posts.
  • Tailor your application and CV to UK conventions.
  • References and a clear, honest account of your experience matter.
  • Be open to location — flexibility widens your options considerably.

Settling in

Expect a structured induction, shadowing, and support with local IT and prescribing systems. Ask questions, learn the escalation pathways, and lean on your team — UK practice is collaborative, and seeking help early is seen as safe, not weak.

Your PLAB 2 preparation pays off

The consultation structure, safety-netting and clear communication you drilled for PLAB 2 are exactly what NHS clinical work rewards day to day. The habits transfer directly. For the bigger picture, see our guide to the full IMG route to UK practice.
